Hepatitis B Notice
The Occupational Safety and Health Administration (OSHA) in accordance with "bloodborne pathogens standard" requires employers to offer three injection Hepatitis B vaccination (HBV) series to all employees who could be exposed to blood or other potentially infectious materials daily as part of their job duties. Questions may be referred to the Employee Benefits Department at 480-541-1302.

For health insurance and other employee benefit plans; our vendors indicate that SSN’s are mandatory to identify individuals enrolling for coverage. Further, a new Mandatory Insurer Reporting law, Section 111 of Public Law 110-173, requires group health plan insurers to report SSN’s in order for Medicare to coordinate payments with other insurance benefits. The SSN’s for you and those of your family may be requested for these purposes. If you have questions about when or why a particular plan requires your social security number, please contact the vendor directly.

COBRA is a health insurance continuation option that is available to employees who leave Kyrene School District and had coverage while employed. Kyrene uses a third party vendor for COBRA Services.

COBRA Information

COBRA gives workers and their families who lose their health benefits the right to choose to continue group health benefits provided by their group health plan for limited periods of time under certain circumstances such as voluntary or involuntary job loss, reduction in the hours worked, transition between jobs, death, divorce, and other life events. Qualified individuals may be required to pay the entire premium for coverage up to 102 percent of the cost of the plan.

COBRA elections are available only on the benefits that an employee had elected prior to terminating employment.

COBRA coverage can only be elected for medical, dental, vision or medical flexible spending accounts, if they were carried by the employee prior to termination.
