COBRA Continuation of Coverage RightsHepatitis B Notice
The Occupational Safety and Health Administration (OSHA) in accordance with "bloodborne pathogens standard" requires employers to offer three injection Hepatitis B vaccination (HBV) series to all employees who could be exposed to blood or other potentially infectious materials daily as part of their job duties. Disclosure of your Social Security number is permitted under Section 7 of the Federal Privacy Act of 1974. For health insurance and other employee benefit plans; our vendors indicate that SSN’s are mandatory to identify individuals enrolling for coverage. Further, a new Mandatory Insurer Reporting law, Section 111 of Public Law 110-173, requires group health plan insurers to report SSN’s in order for Medicare to coordinate payments with other insurance benefits. The SSN’s for you and those of your family may be requested for these purposes. If you have questions about when or why a particular plan requires your social security number, please contact the vendor directly. -

NOTIFICATION: The coronavirus-relief bill known as the Coronavirus Aid, Relief, and Economic Security Act (CARES Act) was signed into law on March 27, 2020. The CARES Act allows 403b optional retirement plan participants greater access to their 403b investments if they are facing financial hardship. Participant questions regarding hardship withdrawals and loans can be directed to AIG Retirement Services at 1-800-569-7055.

COBRA InformationCOBRA gives workers and their families who lose their health benefits the right to choose to continue group health benefits provided by their group health plan for limited periods of time under certain circumstances such as voluntary or involuntary job loss, reduction in the hours worked, transition between jobs, death, divorce, and other life events. Qualified individuals may be required to pay the entire premium for coverage up to 102 percent of the cost of the plan.COBRA elections are available only on the benefits that an employee had elected prior to terminating employment. -

To check to see if your medical provider is currently contracted in the BCBS network, you can click on the link below. Since providers can move in and out of networks at any time, it is recommended that you return to this link to check your providers during the spring open enrollment and again when the plan becomes effective on July 1, 2022.Visit www.azblue.com/chsnetwork to research contracted providers/facilities within Blue Cross Blue Shield Arizona.
