Book Report Form

Please answer the questions on your own paper, using complete sentences and in final copy format.  Remember to defend your answers using examples from the book.  We need to know you understood the book.

1. What is the title? (Book titles are underlined)

2. Who is the author?

3. What is the publishing information? (date, company, place)

4. How many pages are in this book?

5. What is the setting? (Infer as needed)

6. Who is the main character?  Tell something about that character and use examples from the book.

7. Write a summary that includes the who, what, where, when, and why of the story.  The summary must be a progression from the beginning to the end.

8. What is the climax of the story? (The main point and the turning point)

9. What is your evaluation of the book and would you recommend it to a friend?  Be specific. (4-6 sentences)
