Hello Parents/Guardians,

I am sending home the first set of book/software order forms for child to review. Your child will be receiving book order forms approximately monthly. All order forms will be due one week from when your child brings the forms home (if brought home on a Friday, due back to me the following Friday). Please remember that orders are optional – you do not need to place any orders. If you or your child would like to place an order, please do the following:

1. Complete the order form for each pamphlet, including your child’s name on it.

2. Write a check (no cash, please) payable to one of the companies you ordered from (Scholastic, Troll, etc.) for the EXACT amount of your TOTAL order (ALL COMPANIES ORDERED FROM).

3. Staple the check to the order.

4. Place the order AND check in an envelope, seal it, and write on it “Book orders – Mrs. Whitaker” -- this will help to make sure nothing gets lost and if it does, it will have a better chance of getting to me.
