FALL SPORTS
In order to try-out/participate in a fall sport, students must submit the following paperwork to Student Services:
Athletic Partipation Form
Current Physical (less than one calendar-year old)
Participation Fee Agreement
Sports Agreement
Athlete Information Sheet
An academic/grade sheet is not needed for fall sports.
A packet which includes all of these forms will be available in the school office when the office reopens on July 26th.
All paperwork is due: August 13th, 4:00 pm
Soccer/Volleyball Try-Outs begin: August 16th
Cross Country Practice beings: Aug 23rd, 6:30-7:30 am
Parent Meeting: August 30th, 5:00 pm, Library
Soccer/Volleyball Pictures: Sep 1, 1:45 pm
Cross Country Pictures: Sep 1, 7:00 am
SPORTS PHYSICALS
Physico Health Assessments will offer sports physicals on Kyrene campuses at a cost of $25. To schedule an appointment, go to www.physico2go.com. For a list of dates and sites, click here.
PAYING THE ATHLETIC PARTICIPATION FEE
When your child makes a sports team, you have several options to make your participation fee payment:
You can pay on line. Your student should have brought home a packet giving you directions on how to pay on line.
You can call your payment in to the district office. Please call the District Office at 480-783-4060.
You can go to the Kyrene School Districts Customer Service Center at 8700 South Kyrene Rd. Tempe. It is located at the corner of Kyrene and Warner.
Or you can write your VISA, Master Card or Discover card number on the Athlete Information Sheet. Don’t forget to sign the authorization signature line.
Once payment is made please return the Athlete Information sheet with your athlete to Student Services.
All payments are due by the first day of practice.
