CHECKLIST FOR LANGUAGE SAMPLE ANALYSIS
Student_____________________________Date__________School_______________________________
Use this checklist to identify primary areas of weakness from language sample analyses. Analyses should be based on approximately 50 complete and intelligible utterances. Note whether a conversational sample, an extended discourse sample, both was obtained. With the exception of Utterance Formulation (Mazing) characteristics, areas of weakness are indicated when 10% or more of the utterances contain problem characteristics from the middle column. Results from this checklist will be used when determining eligibility for speech/language services. More detailed analyses of problem areas may be required for formulating relevant goals/objectives on students' IEP.
