Please click on the links below to obtain information regarding the Highly Qualified Teacher requirements for Arizona public school teachers.  There are separate attestation forms for Elementary Grades K-6, Middle School Grades 7-8, Special Education, Structured English Immersion (SEI), Reading Specialist, and Title 1 Paraprofessionals.  Please use the appropriate attestation form for the grade level and/or subject area for which the highly qualified status is being requested.
